Claims provisions can vary from state to state. Typically, a notice of claim must be given within 20 days after the date any covered injury, illness, death, or loss occurs or begins or as soon as reasonably possible.

After you give notice, we must, within 15 days in most states, provide claim forms. You must then submit written proof of claim within 90 days of the date of the loss, unless it is not reasonably possible to give written proof within that time.
